I'm using a replacement shell called litestep. This is only one theme of many that you can download (I've modified it a little), but if you have the patience, and can use Photoshop fairly well, creating your own "look" is not that difficult.

well i change mine whenever i get bored, so the pic varies and the items on the left are not very interesting (i only ever use my laptop for work, CC (and i hardly ever come on CC anymore....) ,msn and checking news sites and stuff.) but at the mo the picture i have as my background is
