If only there was some kind of information contained in an appropriately titled thread to read. Perhaps the thread could be made so that it would always be prominent in the forum...'stuck', if you will.

I'm not going to win friends here, but it could be greatly improved with an actual interactive form to fill in. Drop down options, separate text boxes etc. This might have been the best 7 years ago, but it isnt anymore.
